Pfizer has issued a recall of nearly a million birth control pills after a packaging error could cause women to become unintentionally pregnant.

A mix up of placebos and contraceptives could cause the pills to become significantly weakened, potentially increasing the users’ risk of becoming pregnant. The brands affected by the recall are Lo/Ovral-38 and the generic forms, norgestrel and ethinyl estradiol, with expiration dates between July 31, 2013 to March 31, 2014.

Women who believe their birth control package is defective should start using non-hormonal birth control immediately. The mix up is not believed to cause harm to the child should a pregnancy occur.

California Innovations, Inc. and the Consumer Product Safety Commission announced the voluntary recall this week. The product in question is the three-piece insulated and expandable Ci Sport lunch box set, which came with an aluminum bottle and freezer gel pack.

The company received two reports of dogs ingesting the gel from the freezer packs and becoming ill. One dog died as a result.

The lunch boxes were sold in Costco, Leon Korol, and Cost U Less stores from May 2007 to September 2008. Consumers can return the product to Costco for a full refund.

Rescue workers continue to search for the 24 passengers and crew members still missing after a luxury cruise liner crashed into rocks near the Tuscan coast last Friday.

Search efforts were postponed on Wednesday due to rough seas and poor weather conditions. Crews are under added pressure as worse weather is expected to come in as the weekend nears and the threat of the vessel shifting deeper into the sea increases.

So far 11 people have been confirmed dead due to the ship wreck. The captain has come under intense scrutiny for leaving the ship as passengers and crew members tried to escape. He is under house arrest and is facing charges of manslaughter, causing a ship wreck, and leaving before all passengers were evacuated.

A total ban on all cellphone and other portable electronic use while driving was proposed early last week by the National Transportation Safety Board. The proposed restriction comes amid rising numbers of deadly car accidents caused by drivers distracted by their phones.

The NTSB in the past proposed a ban on cellphones for commercial truck and bus drivers, but this law would restrict the use to all drivers and include talking, texting, and emailing.

A fatal accident that occurred last year in Missouri was a major point in pushing the board to suggest the nationwide ban. A teenager sent 11 text messages in the final 11 minutes leading up to the multi-car pileup that killed him and another teen.

Early this morning a woman was killed on her way to work when an elevator began to move as she was walking in. She became trapped between the elevator and the shaft wall as it ascended.

The woman was identified as a 41 year-old employee of the advertising agency Y&R. Two people inside the elevator witnessed her gruesome death and suffered minor injuries. They were taken to the hospital for a psychological evaluation.

The Manhattan office building has a history of safety violations. In June, an inspection found a defect in one of the elevators. The New York City Department of Buildings will conduct an investigation.

Our deepest sympathies are with the family and friends of the victim.

From Saturday to Tuesday there were four house fires in the state of Iowa, one proved fatal and killed an 18 year-old man.

Fire Marshals claim that most fires occur between October and May, and especially during the winter months. Investigators believe the house fire that killed the teenager was caused by a space heater. The smoke alarms in the home were not working at the time the fire broke out.

Firefighters say that 86% of deadly fires are caused because homes or apartments do not have working smoke detectors. The fire marshal’s office donated and installed over 20,000 smoke detectors in homes with young children and an additional 11,000 will be installed free of charge.
